While pregnancy is an exciting time for expectant mothers, it can pose extra health challenges. This is especially true regarding dental health.
Because of hormonal changes that naturally occur during pregnancy, your teeth and gums are at higher risk for dental disease. These changes can increase cravings for carbohydrates, particularly sugar. Increased sugar consumption feeds bacteria found in dental plaque, which is most responsible for tooth decay and periodontal (gum) disease.
Hormonal changes can also make your gums more susceptible to infection. Conditions may be favorable for a form of gum disease called pregnancy gingivitis, which can begin as an infection in the surface layers of the gums. But like other forms of gum disease, pregnancy gingivitis can advance below the gum line and lead to serious health consequences.
Because of this "pregnancy effect" on your teeth and gums, there are some things to which you should pay heed while you're expecting. First and foremost, keep up a daily regimen of brushing and flossing to remove accumulated dental plaque. You should also control your sugar intake to minimize bacterial growth that can cause disease.
It's also important for you to continue regular dental visits during your pregnancy. Your dentist will monitor your dental health and initiate treatment if you begin to show signs of disease. Besides professional cleanings, your dentist may also prescribe antibacterial mouthrinses to combat bacteria.
As far as dental procedures, essential treatments like fillings, root canals or extractions are usually considered safe to perform during pregnancy. But elective treatments of a cosmetic nature are best postponed until after your baby's delivery.
One last tip: because of the higher risk of tooth decay or gum disease, be on the lookout for any abnormal signs in your mouth. This includes spots on the teeth, tooth pain or swollen, reddened or bleeding gums. If you see any of these signs, see your dentist as soon as possible.
Your teeth and gums are indeed at risk for disease during pregnancy. But daily hygiene, regular dental care and attention to signs of disease can help keep that danger at bay.

For years people tuned in to enjoy one of David Letterman's "Top 10 lists," a frequent gag performed on his show Late Night. Each countdown list poked fun at off-the-wall topics like "Top 10 New York City Science Projects" or "Top 10 Questions People Ask when Shopping for an Umbrella."
Recently, the American Dental Association presented their own kind of list—"America's Top 3 Oral Health Problems"—based on surveys of around 15,000 people across the U.S. But unlike the popular Late Night lists, this one is no laughing matter.
Coming in at #3, 29% of the respondents indicated they had experienced tooth pain at some time in their life. Tooth pain is the body's way of alerting to trouble in the mouth, anything from a decayed tooth to a gum abscess. The best thing to do if you have any persistent oral pain is to see your dentist as soon as possible for a thorough examination. And you should do this even if the pain goes away.
The second most prominent oral problem among people is difficulty biting or chewing, about 31% of those in the surveys. As with tooth pain, the reasons can vary greatly, including cracked, loose or deeply decayed teeth, dentures or jaw joint disorders (TMD). Because dental disease is usually the ultimate culprit, the best way to avoid this is to practice daily brushing and flossing and regular dental visits. And, as with tooth pain, you should see your dentist if you're having symptoms.
At 33% of respondents, the number one oral problem in America is chronic dry mouth. It's a constant inadequate flow of saliva often caused by medications or certain systemic conditions. Because saliva helps protect the mouth against infection, a restricted flow increases your risk of disease. If you notice your mouth is dry all the time, you should talk to your dentist about ways to boost your saliva. If you're taking medications, ask your doctor if they could be causing your symptoms and if you could change to something else.
While any of these Top 3 oral problems can be a stepping stone to more serious dental problems, it doesn't necessarily have to lead to that. You can improve your dental health through daily oral hygiene and regular dental treatment. And it might help you stay off this unpleasant list.

While most dental problems are caused by disease or trauma, sometimes the root problem is psychological. Such is the case with bulimia nervosa, an eating disorder that could contribute to dental erosion.
Dental erosion is the loss of mineral structure from tooth enamel caused by elevated levels of acid in the mouth, which can increase the risk for decay and eventual tooth loss. While elevated acid levels are usually related to inadequate oral hygiene or over-consumption of acidic foods and beverages, the practice of self-induced vomiting after food binging by bulimic patients may also cause it. Some of the strong stomach acid brought up by vomiting may remain in the mouth afterward, which can be particularly damaging to tooth enamel.
It’s often possible to detect bulimia-related erosion during dental exams. The bottom teeth are often shielded by the tongue during vomiting, so erosion may be more pronounced on the unshielded upper front teeth. The salivary glands may become enlarged, giving a puffy appearance to the sides of the face below the ears. The back of the mouth can also appear red and swollen from the use of fingers or objects to induce vomiting.
Self-induced vomiting may not be the only cause for dental erosion for bulimics. Because the disorder causes an unhealthy focus on body image, bulimics may become obsessed with oral hygiene and go overboard with brushing and flossing. Aggressive brushing (especially just after throwing up when the tooth enamel may be softened) can also damage enamel and gum tissue.
Treatment must involve both a short — and long-term approach. Besides immediate treatment for dental erosion, a bulimic patient can minimize the effect of acid after vomiting by not brushing immediately but rinsing instead with water, mixed possibly with a little baking soda to help neutralize the acid. In the long-term, though, the eating disorder itself must be addressed. The effects of bulimia are devastating to mental and physical well-being, and no less to dental health. The sooner the disorder can be treated the better the person’s chance of restoring health to their mind, body — and mouth.

As dentists, we often see other mouth problems besides those with teeth and gums. One of the most common is cracking around the corners of the mouth. Although usually not serious, it can be irritating and uncomfortable.
Medically known as angular cheilitis (literally “an inflammation of the angles of the lip”), it’s also called perleche, derived from the French lecher, “to lick.” The latter moniker aptly describes the tendency of sufferers to compulsively lick the sores to relieve irritation, which actually can make things worse.
Perleche has a number of possible causes, mostly from in or around the mouth (although systemic diseases or medications can cause it on rare occasions). It’s often found among younger people who drool during sleep or older people with deep wrinkles along the sides of the mouth that increase the chances of dryness and cracking. Long-term wind or cold exposure, ill-fitting dentures or a lack of back teeth (which help support facial structure) may also contribute to the condition.
Patients with perleche can also develop yeast infections from a strain called candida albicans. The infection can spread through the whole mouth, significantly increasing the chances of physical discomfort.
Treating perleche often involves topical ointments with inflammation-reducing steroids and zinc oxide, which has antifungal properties, to provide an environmental barrier during the healing process. If a yeast infection occurs, we may treat it with oral or topical antifungal medication like Nystatin for the whole mouth and chlorhexidine rinses, which has antibacterial properties.
It also helps to adopt a few preventive measures that can minimize the occurrence of perleche. If you wear dentures, for example, cleaning them often (including, if necessary, with chlorhexidine) and leaving them out at night reduces bacterial and fungal growth. We can also see if your dentures are fitting properly. Replacing missing teeth provides better facial support and could minimize wrinkling around the mouth. And, of course, keeping up daily brushing and flossing helps ensure a healthy and disease-free mouth.

As the old song says, “’Tis the season to be jolly.” And for many of us, the year-end holidays offer a perfect opportunity to break out of our daily routine and get together with co-workers, friends and family. We’re not going to say that you should never indulge in a sugar cookie or a tumbler of eggnog. But everyone knows that too much of a good thing can be bad for your health. So here are some simple tips to help keep your oral health in good shape while you’re enjoying the holiday season.
Choose Healthier Snacks — good-tasting munchies don’t have to be bad for you. Plant-based hors d’oeuvres like hummus with raw vegetables can be just as delicious and satisfying as chips and dip—and a lot healthier, with plenty of vitamins and fiber, and little or no sugar. Cheese, yogurt and other dairy products, eaten in moderation, can actually be beneficial for your oral health: they can stimulate the flow of saliva and restore minerals to the teeth. If you choose to eat sweet snacks, limit them to around mealtimes. That way, your mouth gets a break from sugar and acid in between meals.
Drink Plenty of Water — Sure, there are plenty more exotic beverage choices. But for better health, alternate those fancy drinks with glasses of water. Sugary, acidic beverages like soda (or even juice) can feed decay-causing bacteria and weaken the tooth’s enamel, leading to cavities. Alcohol dries out the mouth, which can cause a number of oral health problems. But water promotes the body’s production of beneficial saliva, and keeps you healthy and hydrated. It also helps neutralize tooth-eroding acid and wash away sticky food residue that can cling to your teeth.
Don’t Neglect Your Oral Health Routine — Sure, between frantic holiday shopping and eagerly anticipated get-togethers, it may seem like there aren’t enough hours in the day. But it’s always important to maintain your regular oral health routine—and even more so at this time of year. Brushing twice a day for two minutes each time and flossing once a day are proven ways to prevent cavities and gum disease. Find a few minutes to take care of yourself and you can keep your smile looking good all year long.
